Waverley Glen has generously donated new slings for Easter Seals Ontario clients and is pleased to offer two programs.
Program 1: New Slings for Easter Seals Ontario families
Eligibility for these slings is on a first come, first served basis and is exclusively for replacement slings.
Waverley Glen produces a variety of slings utilizing an industry-standard hook and loop system and may be used in conjunction with any lifts that utilize a loop-based carry bar design, as long as the following precautions are taken prior to use:
(1) For a sling to be assessed to be compatible with a hoist, a risk assessment should be conducted, either by a qualified caregiver, or through the utilization of the sling and lift manufacturer’s representative.
(2) Initial appraisal of the sling and lift should ensure the following:

That the distance from the carry bar to the sling is appropriate;

That the user should have multiple inclination options; and;
-
That the carry bar of the lift should not constrict the user, nor should it be excessively wide.
Please note, the prescribing therapist must recommend the correct size and type of sling for your child. Upon approval, the sling will be mailed directly to your home. To apply, please complete the attached application form.
Program 2: Waverley Glen Track Lift System Coupons for a Complimentary Sling
This program provides a coupon for a complimentary sling for families who have received funding from Easter Seals Ontario for a Waverley Glen Track Lift System. This coupon will be valid for 2 years from the date the warranty card is sent to Waverley Glen.
The following describes how the program works:

Family applies to Easter Seals Ontario for funding for a Waverley Glen Lift System using The Request for Financial Assistance Application Form. Upon approval, Easter Seals will send the family an approval notification, a coupon for a secondary sling and an order form to be filled out and returned to Waverley Glen to claim their complimentary sling.

Once you have received the lift from Waverley Glen, please complete and send in the Warranty Card for the lift as well as a copy of the approval letter from Easter Seals Ontario. This process allows Waverly Glen to create a customer file for your child and recognize that your family is an Easter Seals Ontario client.

When you wish to redeem your coupon, please fax or mail the coupon along with the completed order form.

Please be aware of the size and type of sling that your child requires. Click here to view a sample order form that will be mailed and a description of the slings and sizes.

Please note, coupons will be valid for 2 years from the date of warranty card activation. Please send in the warranty card within 15 days of installation.

Waverley Glen will send the sling to your home, as well as cover the cost of shipping.
