Families not currently registered with Easter Seals Ontario must apply for registration.
Families registered with Easter Seals Ontario, can obtain an application via download or by contacting the Financial Assistance Program.
The application period for submission of Request for Financial Assistance is January 1 to September 30 annually.
The completed application with accompanying documentation is returned by mail to the Program Coordinator. If there is missing information, the family will be contacted to supply the information.
The application is reviewed based on set criteria.
Staff contact the family to inform them that funding has been approved or not approved. Staff will suggest other sources of funding if not approved.
A vendor authorization form is sent to the vendor identified by the parent on the application form.
The parent is responsible to confirm with their therapist funding is approved and to order the equipment.
At the time of delivery, the parent signs the invoice and the vendor sends the invoice to Easter Seals Ontario for payment.
Two competitive quotes are generally required, except when the item is a highly specialized custom design or in communities where only one vendor is available.
Appealing a request which has been refused
A policy and three-stage appeal process are in place. Parents may initiate an appeal by writing a letter to the Financial Assistance Program. Unresolved appeals advance to the provincial and board level.
Will equipment, services be funded after it has been purchased/obtained?
No, all requests for financial assistance must be approved prior to commitment or purchase.
How long is a request for financial assistance valid?
A request if valid for 4 months from the date of approval. The invoice from the vendor must be received prior to the end of the four months.

Registered families should contact the Financial Assistance Program Coordinator for equipment requests. If the request is for assistance for meals/accommodation/transportation or for other information, please contact the Easter Seals’ office serving the community.
