This is a joint application form with Ontario March of Dimes’ Home and Vehicle Modification Program. This application allows you to complete a portion of the application on-line and you are able to select the option of having your demographic information transferred to the March of Dimes Program, saving you some time in completing their initial screening process. This application is to be completed if applying for home accessibility aids such as a porch lift/ track lift or van lifts.
