The Easter Seal Society, Ontario is considering a new program for youth age 18 and over with physical disabilities, who are facing the tranistion between secondary school and university/college.

The program is currently in the development phase, however we would value your input as to what type of support youth and families need during this time of transition.

To help us with the development of this program, we would greatly appreciate if you would take a few minutes to complete a survey. There is one survey designed for parents/service providers and a second survey specifically for youth.
